Family Medicine Consultant – Diabetes Specialist

About the role

This position provides comprehensive medical care to adult patients in both in‑person clinic and telemedicine settings, with a primary focus on diabetes management within family medicine. The consultant will lead diagnosis, treatment, and long‑term optimization of metabolic disorders while ensuring evidence‑based, patient‑centred care across all channels.

Key responsibilities

• Conduct thorough clinical evaluations, including history, physical or virtual assessment, medication review, and interpretation of laboratory results.
• Determine when patients require an in‑person visit versus virtual management and facilitate seamless transitions.
• Develop individualized, evidence‑based management plans covering glycemic control, lifestyle modification, preventive care, and specialist referrals.
• Collaborate with endocrinologists, dietitians, diabetes educators, pharmacists, and primary care providers to deliver coordinated care.
• Utilize digital health technologies, remote patient monitoring, and telemedicine platforms to maintain high‑quality care and accurate documentation.
• Promote population‑health and value‑based initiatives to improve outcomes and optimize resource utilization.

Required profile

• Medical degree with board certification or equivalent in family medicine.
• Proven experience in diagnosing and managing type 1, type 2 diabetes, pre‑diabetes, obesity, and related metabolic conditions.
• Ability to deliver care effectively in both face‑to‑face and virtual environments.
• Strong communication skills for patient and family education.
• Commitment to staying current with diabetes guidelines and quality‑assurance standards.
