About the role

The Registered Social Worker at Extendicare Eaux Claires provides psychosocial support to residents and families in a 204‑bed long‑term care home. The role involves assessing emotional and social needs, developing care plans, and collaborating with multidisciplinary teams to enhance quality of life.

Key responsibilities

• Respond to inquiries about placement, lifestyle, policies, and procedures.
• Conduct tours of the home for applicants and their advocates.
• Manage pre‑admission procedures, including application screening and assessments.
• Coordinate admissions and accompany new residents and families through the process.
• Advocate for residents and families, promoting a resident‑oriented environment.
• Plan, organize and deliver social work programs addressing psychological needs.
• Participate in multidisciplinary care planning and resident care conferences.
• Facilitate positive resident groups and organize family orientation programs.
• Provide education and consultative services to staff, residents, and families.
• Document statistical reports for quality management and policy development.
• Uphold ethical standards and work with vulnerable adults in a position of trust.

Required profile

• Registered Social Worker credential.
• Strong communication skills and empathy.
• Knowledge of aging, healthcare systems, and discharge planning.
• Ability to work collaboratively with multidisciplinary teams.
• Commitment to ethical practice and resident advocacy.
• Experience working with vulnerable adult populations.
