Care Coordinator – Patient Wellness & Preventive Care

About the role

We are seeking a Care Coordinator to ensure seamless patient care by coordinating medical services and promoting wellness and preventive health. The role serves as the central point of contact between clients, healthcare providers, and community resources, driving person‑centered care.

Key responsibilities

• Engage clients to educate them on regular check‑ups, screenings, vaccinations and healthy lifestyle choices.
• Collaborate with physicians and specialists to develop personalized, prevention‑focused care plans.
• Conduct health assessments, identify risk factors and coordinate appropriate screenings (e.g., hypertension, diabetes, cancer).
• Arrange referrals to specialists and preventive services, ensuring smooth transitions of care.
• Maintain clear communication among all parties, document interactions, and monitor adherence to care plans.
• Track client progress, address barriers, and provide ongoing support to reinforce preventive measures.
• Build relationships with community partners to expand access to preventive resources.
• Prepare reports and documentation required by healthcare regulations and internal quality initiatives.
• Adapt to work across multiple sites and perform additional duties that support the organization’s wellness objectives.

Required profile

• Strong understanding of preventive care principles, including screenings, vaccinations, lifestyle modifications and health promotion.
• Commitment to person‑centered care values and continuous improvement.
• Ability to maintain strict confidentiality and adhere to safety, cybersecurity and ethical standards.
• Flexibility to work across different locations and collaborate with multidisciplinary teams.
• Completion of mandatory education and training as required by the employer.
