Patient Experience Coordinator

About the role

The Patient Experience Coordinator supports safe, timely transitions for patients discharged from acute care and acts as a patient advocate. Working under the supervision of a manager and clinical providers, the role focuses on triaging needs, providing education, and resolving complaints to improve overall satisfaction.

Key responsibilities

• Coordinate assessments and patient education for newly discharged patients.
• Receive and investigate patient and family complaints, ensuring resolution and closure.
• Serve as a resource for medication information, follow‑up appointments, test results, and treatment details.
• Lead patient rounds, prioritize revisits, and oversee service‑recovery initiatives across hospital departments.
• Map processes, shadow patient journeys, and identify improvement opportunities.
• Analyze patient satisfaction surveys, complaints, and variance data to recommend enhancements.
• Collaborate with physicians, leadership, and staff to address cultural diversity and service‑related issues.

Required profile

• Minimum of 1 month of relevant experience in patient experience, service recovery, or a related healthcare role.
