Case Manager – Patient Care Coordination

About the role

The Case Manager coordinates the care of patients undergoing surgery, from pre‑operative preparation through discharge, ensuring high‑quality care and patient satisfaction. This role involves direct communication with patients, families, and multidisciplinary clinical teams.

Key responsibilities

• Verify consent forms, pre‑anesthetic assessments and educate patients about the surgical process via face‑to‑face interaction and SMS.
• Conduct comprehensive pre‑operative assessments, including medical history, medications, allergies and psychosocial factors.
• Collaborate with surgeons, anesthesiologists and nursing staff to develop individualized peri‑operative care plans.
• Manage referrals, extra payments and device availability to prevent surgery cancellations.
• Maintain detailed records in compliance with organizational policies and regulatory requirements.
• Resolve operating‑room delays by communicating promptly with relevant parties.
• Initiate discharge planning at admission, identify post‑acute needs and arrange rehabilitation, home health and follow‑up appointments.
• Act as patient advocate, addressing concerns, barriers and connecting families to community resources.
• Participate in quality‑improvement initiatives related to pre‑operative processes and discharge planning.

Required profile

• Experience coordinating patient care in a surgical or hospital setting.
• Strong knowledge of peri‑operative workflows, pre‑operative assessments and discharge planning.
• Excellent communication skills for interacting with patients, families and multidisciplinary teams.
• Ability to maintain accurate documentation and comply with healthcare regulations.
• Proactive problem‑solving attitude to address delays and barriers to care.
