Consultant – Family Medicine (Geriatrics)

About the role

The Consultant – Family Medicine (Geriatrics) provides comprehensive care for older adults through both in‑person clinic visits and virtual telemedicine consultations. The role focuses on the unique medical, functional, cognitive, and psychosocial needs of the geriatric population, ensuring continuity of care across all settings.

Key responsibilities

• Conduct thorough clinical evaluations, including history taking, physical or virtual assessment, medication review, and geriatric assessment when appropriate.
• Diagnose and manage acute and chronic conditions, addressing multimorbidity, frailty, dementia, and polypharmacy.
• Determine the appropriate modality (in‑person vs. virtual) for each patient and facilitate seamless transitions between them.
• Develop individualized, evidence‑based management plans that include medication optimization, preventive care, lifestyle interventions, and referrals.
• Collaborate with multidisciplinary teams, caregivers, and community services to deliver integrated, patient‑centered care.
• Utilize digital health technologies and maintain accurate clinical documentation in both settings.
• Promote healthy aging through population‑health initiatives and value‑based care principles.

Required profile

• Board‑certified or eligible in Family Medicine with substantial experience in geriatrics.
• Proven ability to manage complex, multimorbid older patients, including frailty and cognitive impairment.
• Strong communication skills for interacting with patients, families, and multidisciplinary teams.
• Commitment to continuous professional development and staying current with geriatric best practices.
