Senior Associate - Care Coordination

About the role

The Senior Associate – Care Coordination will guide members through their healthcare journey, ensuring they receive coordinated and clearly communicated care across our clinical programmes. Acting as the primary point of contact, you will safeguard continuity between patients, providers and internal teams to help members navigate approved services smoothly and achieve better health outcomes.

Key responsibilities

• Serve as the main liaison for members enrolled in clinical programmes, guiding them through approved care pathways, requirements and next steps.
• Coordinate internally to process and follow‑up healthcare service requests—including appointments, referrals and approvals—from initiation to completion.
• Communicate clearly with members about medical service requests, approvals and programme enrolment status, translating medical terminology into patient‑friendly language.
• Collaborate with healthcare providers, patients and families to facilitate effective care management and improve outcomes.
• Track patient milestones, monitor enrolment and follow‑up schedules, and assist with multidisciplinary interactions.
• Maintain accurate documentation of all interactions, case notes and programme activities in internal systems, ensuring confidentiality and compliance.
• Address patient concerns related to care navigation, identify recurring operational issues and provide feedback for process improvement.

Required profile

• Degree in Nursing, Health Sciences, Public Health or a related healthcare field.
• Strong understanding of medical terminology and basic clinical pathways.
• Familiarity with healthcare authorisation and service request processes.
• Excellent verbal and written communication skills.
• Ability to manage multiple cases simultaneously with attention to detail.
