About the role

The Care Coordinator ensures seamless patient care by coordinating medical services, promoting wellness, and supporting preventive health initiatives. Acting as the central point of contact, the role bridges clients, healthcare providers, and community resources to optimize health outcomes.

Key responsibilities

• Engage with clients to educate on regular check‑ups, screenings, vaccinations and healthy lifestyle choices.
• Collaborate with providers to develop personalized wellness and preventive care plans.
• Conduct health assessments, identify risks and coordinate appropriate screenings (e.g., hypertension, diabetes, cancer).
• Arrange referrals to specialists and preventive services based on client needs.
• Maintain clear communication among clients, providers and other professionals, ensuring accurate documentation and smooth transitions of care.
• Monitor client adherence to preventive care plans, follow up on barriers and reinforce importance of screenings.
• Build relationships with community partners to expand access to preventive resources.
• Keep comprehensive records and prepare reports required by healthcare regulations.
• Adapt to work across multiple sites and perform additional duties supporting wellness objectives.

Required profile

• Strong understanding of preventive care principles, including screenings, vaccinations and health promotion.
• Ability to stay current with guidelines and best practices.
