Nutrition Therapist – Home Care

About the role

The Nutrition Therapist provides personalized clinical nutrition services directly in patients' homes, helping them achieve health goals such as weight management, healthier eating habits, and stress reduction. The role combines patient education, nutritional assessment, and coordinated care with multidisciplinary teams.

Key responsibilities

• Visit patients at home to deliver clinical nutrition therapy according to scheduled appointments.
• Prepare and maintain a medical bag with necessary supplies for each visit.
• Ensure all equipment and tools are sterilized before and after each session.
• Conduct comprehensive nutritional status assessments and develop individualized care plans.
• Provide age‑specific nutrition counseling that respects cultural and clinical needs.
• Order diet supplements and manage enteral nutrition in line with Dallah Health policy.
• Document subjective and objective findings, assessments, and treatment plans in clinical notes, including discharge summaries.
• Participate in multidisciplinary team meetings to integrate nutrition care with overall patient treatment.
• Engage in safety and risk management programs, report incidents, and contribute to incident reviews.
• Educate patients and families to promote understanding and active participation in care.

Required profile

• 1–2 years of experience in clinical nutrition or related field.
• Bachelor of Science in Nutrition and Dietetics.
• Valid medical license for clinical nutrition practice.
